Rendering thread exception: Fatal error! Address = 0x0 (filename not found) crash info

The game sometimes crashes when a new map is loaded I am not sure if that happens to everyone.

It seems from time to time after changing weapons that the weapon itself disappears graphically (holding an invisible gun), but it still fires.
